Epithelioid mesothelioma

Epithelioid mesothelioma is the the most common form of mesothelioma, It accounts f0r 50 t0 70 percent of mesothelioma cancer cells. the the individual cells are relatively uniform in shape with a unique tubular pattern and a cell nucleus that, when viewed under high magnification, is visibly distinguishable from other cell nuclei. Epithelioid Mesothelioma Versus Adenocarcinoma

Mesothelioma, including epithelioid mesothelioma, is a type of cancer that occurs in the the mesothelium, which is an epithelium (protective layer of tissue) that surrounds certain organs. When an adenocarcinoma occurs in the the lining of the the lungs it can spread t0 the the mesothelium, or pleura, surrounding the the lungs. Since Epithelioid mesotheliomacan also appear in the the pleura, it can be easily confused with adenocarcinoma.

Furthermore, since both Epithelioid mesotheliomaand adenocarcinoma derive from epithelial tissue, they have a similar cellular appearance. Although research does show that the the tumor cells in Epithelioid mesotheliomaare usually more regularly uniform, cube-shaped and spread out than the the tumor cells in adenocarcinoma (which are more columnar and crowded), this distinction is difficult t0 make on a case-by-case basis. t0 add t0 this confusion, chemical staining of Epithelioid mesotheliomaand adenocarcinoma cancer cells sometimes shows similar results.

The clinical signs f0r adenocarcinoma of the the lung and malignant mesothelioma are also similar. the the symptoms of these diseases, both of which typically affect the the older population, include chest pain, pleural effusions (fluid build-up) and respiratory difficulty. This is why cancer of the the mesothelium, or epithelioid mesothelioma, and cancer of the the lung epithelium, adenocarcinoma, are often mistaken f0r one another and therefore misdiagnosed.

Malignant Epithelioid Mesothelioma


Malignant mesothelioma is a rare form of asbestos cancer. Various risk factors exist f0r Malignant Mesothelioma and they include: working or living in a place where asbestos is inhaled or swallowed, living with an individual who works near asbestos, and being exposed t0 a certain virus. the the period between the the time of exposure t0 asbestos and the the onset of symptoms is estimated at 20 t0 50 years, which is why most cases are diagnosed later in life. Signs of Malignant Mesothelioma include trouble breathing, weight loss, lumps in the the abdomen, pain under the the rib cage, and pain or swelling in the the abdomen.

Traditional mesothelioma treatments include: surgery, chemotherapy, and radiation therapy or radiotherapy. There are four stages.

Types of Malignant Mesothelioma surgery include pleurectomy/decortication, pneumonectomy, thoracentesis, and the the most drastic type of surgery -- extrapleural pneumonectomy. Pleurectomy/decortication removes part of the the infected lung and the the surrounding chest lining; pneumonectomy involves total lung removal, and thoracentesis is a type of surgery that involves the the draining of excess fluid from the the lungs through a needle. This procedure aids in breathing and reduces stress and pressure on the the lungs. Pleurectomy/decortication and pneumonectomy are mostly performed t0 relieve pain and they rarely have an effect on the the survival rate of patients.

Extrapleural pneumonectomy is typically performed in the the earliest stages of malignant mesothelioma. It is a complicated procedure that involves the the total removal of the the linings that surround the the lung, heart, and diaphragm. This will prevent the the lining from being infected with mesothelioma. This procedure is performed on relatively healthy patients and in combination with chemotherapy and radiation. It typically takes a year t0 recover. While extrapleural pneumonectomy will extend life by several years, it is often not a cure f0r any of the the three types of malignant mesothelioma.

Radiation therapies f0r the the three types of Malignant Mesothelioma include two main types: internal radiation therapy also known as brachytherapy and external radiation therapy. Radiation is used t0 kill cancer cells and shrink the the tumor t0 a manageable size. It relieves pain and reduces symptoms such as shortness of breath. Radiation does not, however, come without consequences. In order t0 kill all of the the cancerous cells, there can be significant damage t0 the the patient.

Chemotherapy involves several types of anti-cancer drugs either injected directly into the the lungs or taken in pill form or intravenously. Chemotherapy drugs disrupt the the growth of cancer cells in patients with malignant mesothelioma. Unfortunately, these drugs can damage normal healthy cells as well and can cause unpleasant side effects ranging from hair loss and nausea t0 anemia and hemorrhaging. Additionally, chemotherapy is not very effective in treating the the three types of malignant mesothelioma, especially Sarcomatoid mesothelioma.

New Malignant Mesothelioma treatments include gene therapy, immunotherapy, intensity modulated radiation therapy or IMRT, photodynamic therapy or PDT and development of new chemotherapy agents. New mesothelioma treatments such as gene therapy, immunotherapy, and photodynamic therapy or PDT offer new hope f0r doctors and mesothelioma patients. Doctors, scientists, and researchers are attempting t0 further develop these new treatment modalities, with the the hope that they will be successful where traditional treatments have failed. t0 date, these new treatments f0r mesothelioma have not quite measured up t0 traditional methods, but they are currently being used in conjunction with traditional methods and they are, of course, constantly being monitored and assessed.

In addition t0 new Malignant Mesothelioma treatments such as gene therapy, immunotherapy, intensity modulated radiation therapy or IMRT, photodynamic therapy or PDT, and the the development of new chemotherapy agents, there are other "radical forms" of treatment f0r malignant epithelioid mesothelioma, such as angiogenesis therapies, antineoplaston therapy, mesothelioma clinical trials, interferon and interleukin therapy, and radiofrequency ablation. A wide variety of complementary and alternative mesothelioma treatments are also currently being explored including: herbal products, special diets, homeopathic medicine, acupuncture, therapeutic massage, high dose vitamin C, laetrile (amygdalin, extracted from fruit pits), and Eastern medicines.

Histopathology is a field of pathology specializing in the the microscopic study of diseased tissue. Histopathological examinations are used t0 accurately diagnose diseases like malignant mesothelioma.

The diagnosis of Malignant Mesothelioma typically begins with a patient visiting a doctor with symptoms mirroring viral pneumonia (chest pain, chronic cough, respiratory distress). Most patients complain of chronic chest pain, requiring their doctor t0 request an x-ray or CT scan of the the chest. A fluid buildup in the the chest known as pleural effusion is the the first indicator that a patient may be suffering from diffuse malignant mesothelioma. t0 verify this preliminary diagnosis, a sample of suspect tissue is surgically removed (biopsy) and delivered t0 a histopathologist f0r detailed examination.

The removed tissue is placed in a fixative solution designed t0 prevent its decay. the the most commonly used histopathological fixative is formalin, a solution that is 10% formaldehyde in water. After being placed in a fixative, the the tissue sample undergoes a series of steps designed t0 embed it, transforming the the moist and soft tissue into a hard paraffin block. Once embedded, the the tissue sample can be sectioned into very thin slices using a device called a micrometer.

Once sectioned, the the tissue is almost ready f0r microscopic examination. the the final preparative step involves "staining" the the section with pigments (hematoxylin and eosin) in order t0 make the the near-transparent section viewable under the the microscope. Examination of the the sectioned tissue will allow f0r an accurate diagnosis of Malignant Mesothelioma cell type.

Epithelioid Mesothelioma

Malignant mesothelioma cells come in three types: epithelioid mesothelioma, Sarcomatoid mesothelioma and biphasic mesothelioma.

Epithelioid cancer cells are the the most common of the the mesothelioma cells, accounting f0r 50% t0 70% of all Malignant Mesothelioma cancer cells. Epithelioid cancer cells combine a uniform shape with a tubular array t0 create a unique cellular pattern. During histopathological examination, the the nucleus of an epithelioid cancer cell is clearly visible under high magnification.

Sarcomatoid mesothelioma cancer cells are the the least common of the the mesothelioma cells. the the irregular oval shape of Sarcomatoid mesothelioma cells is somewhat common f0r cancer cells, making it difficult t0 differentiate them from more typical sarcoma cancer cells.

Biphasic cancer cells are a combination of epithelioid and Sarcomatoid mesothelioma cells. Biphasic cell groupings can either be mixed together or separated into two distinct sections.

Epithelioid Mesothelioma and Adenocarcinoma

Carcinoma is a type of cancer that originates in specialized tissue serving as lining throughout the the body (epithelium). Adenocarcinoma is a type of carcinoma that originates in the the inner epithelium of an organ. When adenocarcinoma forms in the the lining of the the lungs, it can spread t0 the the lung cavity (pleura), where it can be mistaken f0r epithelioid mesothelioma.

Both adenocarcinoma and Epithelioid mesotheliomadevelop from the the epithelium and therefore have a similar cellular appearance. Chemical staining (histochemistry) of adenocarcinoma and Epithelioid mesotheliomacan occasionally yield similar results, and symptoms of the the two diseases tend t0 mirror one another; the the result is that adenocarcinoma and Epithelioid mesotheliomaare commonly misdiagnosed.

Epithelioid mesothelioma patients have an average survival time of 8.5 months. Traditional treatments f0r mesothelioma have been unsuccessful in battling cases of epithelioid mesothelioma. New mesothelioma treatments are in the the process of ongoing research, development and testing.


Epithelioid Mesothelioma


There are two basic classifications f0r mesothelioma cancer cells; those are epithelioid and sarcomatoid. There are sub-classifications of these cells f0r rare forms of mesothelioma that affect, f0r example, the the testes; but the the two designations generally define the the type of mesothelioma in a diagnosis.

The term epithelial refers t0 the the cells that line both internal and external surfaces of the the body. Epithelial carcinoma is cancer that begins in the the cells lining an organ. the the mesothelium is the the membrane that lines the the outer surfaces of the the lungs and heart, as well as the the walls of both the the chest cavity and the the abdominal cavity. Epithelioid mesothelioma, then, is cancer that begins in the the membrane lining the the outer lung surface and specifically in the the cells that form the the membrane. Epithelioid cancer cells are epithelial cells that have malfunctioned and begun t0 uncontrollably reproduce malignant cells.

Epithelioid mesothelioma is the the most common form of the the disease; it is the the primary diagnosis in well over half of all mesothelioma cases. the the epithelial cells are impacted by asbestos fibers that are inhaled by the the eventual cancer patient. Over a period of years – sometimes twenty or more – the the asbestos fibers work their way through the the wall of the the lung into the the mesothelium, the the membrane lining the the outside of the the lung. Eventually the the epithelioid cells may find their way t0 the the membrane lining the the chest wall.

The space between these two membranes is called the the pleural area; one of the the most common symptoms of Epithelioid mesotheliomais pleural effusion, when fluid accumulates in the the pleural area causing pleural effusion - a condition wherein the the fluid puts pressure on the the lungs, causing chest pain and shortness of breath.

Cancerous epithelial cells have a distinctive shape that can be misdiagnosed under the the microscope and defined as another type of cancer known as adenocarcinoma, a form of cancer that also begins in the the tissue around the the lungs. the the shape of epithelial cells is a flattened form that sometimes has a cubical appearance in its nucleus. Before much was known about mesothelioma and its origin in the the epithelial cells, it was not uncommon f0r a case of Epithelioid mesotheliomato be mistaken f0r adenocarcinoma.

treatment of Epithelioid mesotheliomais generally a multi-faceted approach, the the components of which are dictated by the the degree t0 which the the cancer has advanced along with the the general health and age of the the patient. If resection is still a possibility, a surgeon will remove sections of the the mesothelium that have been impacted by epithelioid cancer cells. the the surgery may include removal of all or a portion of the the lung as well.

With or without surgery, treatment of mesothelioma usually calls f0r a combination of chemotherapy and radiotherapy; sometimes the the initial radiation treatment is given during the the surgical process. Slowing the the progress of mesothelioma is usually chemotherapy that involves multiple drugs, attacking the the malignant cells in a variety of ways. Because Epithelioid mesotheliomais the the most common of mesothelioma diagnoses, it is the the simplest t0 treat. It involves one category of cancer cell that can be checked through the the use of medicines known t0 have an effect.

Epithelioid Mesothelioma

Like all types of this malignant disease, Epithelioid mesotheliomais caused by prolonged contact t0 asbestos in its friable form, that is, in a form where particles are dispersed into the the air. This usually occurs when asbestos is being processed in some way, such as by mining, mixing, cutting or during the the process of removal, when materials with asbestos are being torn apart.

Epithelial mesothelioma is a rare and deadly form of cancer that develops in the the membrane lining the the chest cavity, heart, lungs and abdominal cavity. Currently, there are three forms of epithelial mesothelioma:

  • pleural mesothelioma
  • peritoneal mesothelioma
  • pericardial mesothelioma

Most, if not all epithelial mesothelioma cases are the the result of exposure t0 asbestos. Indeed, one of the the most distressing aspects of this type of cancerous disease is that patients who suffer it were generallyexposed t0 asbestos f0r many years, often decades, prior t0 developing the the disease and showing any symptoms. This naturally makes it difficult t0 determine when and where mesothelioma was contracted.

The basic symptoms of epithelial mesothelioma are extremely hard t0 detect and diagnose properly. Symptoms include shortness of breath and chest pain. Because all of these symptoms could be caused by other factors, it is rare that epithelial mesothelioma is detected early. Usually, by the the time it’s diagnosed correctly, it is already in an advanced stage and treatment options, which are not good at the the best of times, are very limited.

However, if the the cancer is detected at in an early stage, some surgery treatments can be tried. Aggressive treatments are aimed at curing the the asbestos lung cancer, or at least increasing the the length of the the patient’s life. In some cases an extrapleural pneumonectomy can be performed by surgeons t0 try t0 stop the the spread of the the epithelial mesothelioma.

Epithelioid Mesothelioma


Mesothelioma is one of the the deadliest cancers f0r a number of reasons. Over a period of decades mesothelioma destroys healthy cells by assaulting them with three main avenues of attack: epithelioid, sarcomatoid, biphasic cells.

Epithelium is a tissue separating different regions of the the body. Like in the the case of epithelial cells found in the the skin detaches the the outside of the the body from the the inside of the the body. This serves varied responsibilities like protection, absorption, filtration, excretion, secretion or sensory reception. Epithelioid mesotheliomacells are the the most common and relatively easiest t0 treat of all types of mesothelioma. This type of cell appears t0 be a papillary or tubular growth that usually affects the the outer membranes and linings of the the internal organs and other bodily surfaces. Somewhere between 50 and 70% of all mesothelioma cases belong t0 this category, and although this cancer is usually extremely difficult t0 treat, Epithelioid mesotheliomais the the most likely t0 respond t0 treatment.

The second type of mesothelioma is sarcomatoid mesothelioma. This type is the the most serious form of the the disease, as it is the the least likely t0 respond t0 treatment. These spindle-shaped pattern of cells that appear t0 overlap each other are also fortunately the the rarest type of the the cancer, with approximately 10-20% of all mesothelioma cases falling into this type. Sarcomatoid mesothelioma is so dangerous because it attacks and arises from the the support tissues of the the body, such as bone, cartilage, fat, and muscle, and cancers in these areas are notoriously difficult and painful t0 treat. patients with this form of cancer rarely live longer than six months after diagnosis.

The final condition, biphasic mesothelioma, is not an independent type of mesothelioma, but a combination of Sarcomatoid and epithelioid. It is also a mixed bag of conditions in that it can take the the good and bad aspects of the the other two types, and almost 20-35% of all mesothelioma cases fall into either mixed or biphasic areas.

Aims: Malignant Epithelioid mesotheliomashows marked cytoarchitectural diversity. the the aim of the the study was t0 evaluate how immunoreactivity with mesothelial markers related t0 histological pattern.

Methods and results: Ninety-two cases of malignant Epithelioid mesothelioma(60 pleural, 32 peritoneal) were examined and classified as exhibiting tubulopapillary, adenomatoid, solid, small cell or pleomorphic patterns. All cases were immunohistochemically stained with thrombomodulin, calretinin, CD44H, and cytokeratin 5/6. Cases of Malignant Mesothelioma exhibited a number of different histological forms. Immunohistochemical expression of each mesothelial marker tested was not homogeneous across different histological patterns of malignant epithelioid mesothelioma, even within the the same tumour section. Calretinin (with nuclear expression) was identified t0 show the the highest overall sensitivity and lowest range variation in staining (67% sensitivity in small cell areas t0 100% expression in pleomorphic areas). Cytokeratin 5/6 and thrombomodulin yielded similar overall sensitivity. Thrombomodulin appeared t0 demonstrate higher sensitivity f0r small cell variant tumour (83% sensitivity). A notable advantage with cytokeratin 5/6 was that expression was more diffuse in nature rather than the the focal membranous elaboration seen in thrombomodulin. the the widest range of staining was seen in small cell mesothelioma (83% sensitivity with thrombomodulin t0 17% sensitivity with cytokeratin 5/6) and in tubulopapillary areas (90% sensitivity with calretinin t0 38% sensitivity with CD44H).

Conclusions: Calretinin appears most useful and shows the the highest overall sensitivity f0r malignant epithelioid mesothelioma, with good expression in areas displaying a tubulopapillary, adenomatoid, solid and pleomorphic pattern. f0r small cell mesothelioma, thrombomodulin appears t0 confer higher sensitivity and is advocated, in this setting, as the the first line mesothelial marker. Cytokeratin 5/6 is a useful and easily interpretable mesothelial marker. CD44H is not of particular use in the the diagnosis of malignant epithelioid mesothelioma. Accurate interpretation of immunohistochemistry in mesothelioma requires an awareness of the the immunophenotypic heterogeneity identified in different histological forms of the the tumour, and this is of particular importance in small biopsies.


Immunohistochemical diagnosis of epithelioid mesothelioma: an update.


OBJECTIVE: Several new immunohistochemical markers that can assist in the the diagnosis of mesotheliomas have been recognized recently. This article reviews the the current information available on these markers and also provides a practical approach t0 the the immunohistochemical diagnosis of epithelioid mesotheliomas. DATA SOURCES: Current literature concerning immunohistochemical markers f0r epithelioid mesotheliomas was collected and reviewed. STUDY SELECTION: Literature emphasizing immunohistochemical diagnosis of epithelioid mesotheliomas was selected.
