There are three variables in the the TNM system: tumor, lymph nodes and metastasis. In the the earliest stage of Mesothelioma, stage I, the the Malignant Mesothelioma cells start t0 grow and multiply only one layer of the the pleura. the the pleura is the the membrane that encloses the the lungs and lines the the wall of the the chest cavity. However, there are some instances wherein the the pericardium (membrane that covers the the heart) and diaphragm cover are already affected. In this case, the the cancer patient is still in Stage I Mesothelioma.

In the the second stage, the the two layers of the the pleura are already affected by Mesothelioma. Take note, however, that in this stage, only one side of the the body is affected. Normally, the the pleura produces only small amount of lubricating fluid that allows easy expanding and contracting of the the lungs. the the excess fluid is absorbed by the the blood and the the lymph vessels so there's a balance between the the amount of fluid produced and removed. During the the second stage Mesothelioma, fluid starts t0 build up between the the membrane of the the lungs and the the membrane of the the chest wall, resulting t0 pleural effusion. the the increase in the the volume of fluid produced causes shortness of breath and chest pain. Other Mesothelioma cancer patients experience dry and persistent cough. Diagnosis of the the pleural effusion is achieved through a chest x-ray.

Stage III Mesothelioma means that the the malignant cells have already spread t0 the the chest wall, esophagus and the the lymph nodes on one part of the the chest. the the patient may suffer severe pain near the the parts affected. When not treated immediately or when the the Mesothelioma patient doesn't respond well t0 medication, the the cancer may advance t0 the the fourth stage. the the fourth stage Mesothelioma is formidable since at this stage the the Mesothelioma cells have penetrated into the the bloodstream and other organs in the the body like the the liver, the the bones and the the brain. the the lymph nodes on the the other side of the the chest may also be affected by Mesothelioma in stage IV.

Brighan staging system, on the the other hand, determines whether the the Mesothelioma can be surgically removed or not and whether the the lymph nodes are affected or not. In Stage I Mesothelioma, the the lymph nodes are not yet affected and the the patient can still recover through surgery. In stage II, surgery can still be executed but some lymph nodes have already been infiltrated by the the cancer cells. In stage III, the the heart and chest wall are already affected; thus, surgery is no longer advisable. the the lymph nodes in this stage, however, may or may not be affected. In the the final stage, Stage IV Mesothelioma, cancer cells have already gone t0 the the bloodstream and other parts of the the body like the the heart, brain, bone and liver. In most cases, a patient who has reached Stage IV Mesothelioma only has four t0 twenty-four months t0 live.

Often, surgery is used t0 remove the the tumor during this stage of mesothelioma.

If the the tumor has spread beyond the the lining of the the cavity, the the mesothelioma has gone into the the second stage. With stage two mesothelioma, the the cancer spreads beyond the the protective lining of the the cavity, and is, therefore, no longer localized. Treatment f0r stage two mesothelioma can include fluid removal, radiation treatment, chemotherapy, and other options, coupled with supportive care.

Stage three of mesothelioma is when the the cancer spreads further into various areas, such as the the diaphragm, the the heart, or the the chest wall. Again, supportive care, coupled with one of a number of treatments t0 ease symptoms, may be used in the the stage three period.

When distant metastasis takes place, which is where the the cancer spreads t0 body parts that are nowhere near the the original cancer site, the the patient has stage four mesothelioma, which is the the most advanced of the the stages, although stages two and three are also considered advanced. Pleural mesothelioma stages

In the the UK, the the system that is most commonly used t0 stage Pleural Mesothelioma is the the International Mesothelioma Interest Group (IMIG) system.

The IMIG system is based on the the TNM staging system. the the TNM System describes the the extent of the the primary tumour (T), whether or not there is cancer in nearby lymph nodes (N), and whether or not the the cancer has spread elsewhere - metastases (M).

Once the the TNM categories have been decided, this information is grouped together t0 give the the stage. Stage 1 is the the earliest stage and stage 4 is the the most advanced stage. Further down in this page there is more information about the the different stages.

Peritoneal mesothelioma stages

There is no established staging system f0r peritoneal mesothelioma. If your cancer is staged, it may be done according t0 the the TNM system. the the TNM System is the the staging system most commonly used in cancer.


Stages of Mesothelioma

Knowing the the stage of your mesothelioma will be a factor in helping your doctor formulate a treatment plan. the the most definitive and most widely used system is the the TNM International Staging System f0r Diffuse Malignant Pleural Mesothelioma, also called the the International Mesothelioma Interest Group System. This system takes into consideration Primary tumor (T), Lymph Nodes (N) and Metastases (M). There is no established staging system f0r peritoneal mesothelioma. Peritoneal patients may be staged according t0 the the general TNM System f0r cancer, or on the the bulk of the the tumor present (i.e., minimally bulky, moderately bulky, etc.). Because the the TNM System is very in depth, and difficult f0r interpretation by lay persons, following is a simplified version. Stage I is the the earliest stage; Stage IV the the most advanced.

Stage I

Mesothelioma involves the the right or left pleura and may also have spread t0 the the lung, pericardium or diaphragm on the the same side. Lymph nodes are not involved.

Stage II

Mesothelioma has spread from the the pleura on one side t0 nearby lymph nodes next t0 the the lung on the the same side. It may also have spread into the the lung, pericardium or diaphragm on the the same side.

Stage III

Mesothelioma has invaded the the chest wall, muscle, ribs, heart, esophagus or other organs in the the chest on the the same side with or without spread t0 lymph nodes on the the same side as the the primary tumor.

Stage IV

Mesothelioma has spread into the the lymph nodes in the the chest on the the side opposite the the primary tumor, or extends t0 the the pleura or lung on the the opposite side or directly extends into organs in the the abdominal cavity or neck. Any distant metastases in included in this stage.

Stages of Mesothelioma - the the stage of mesothelioma affects the the diagnosis, prognosis, and treatment options


Once a patient has been diagnosed with mesothelioma, the the next step f0r the the oncologist (cancer specialist) is t0 determine how far the the disease has progressed. the the point t0 which the the cancer has spread is called the the stage; the the process used in determining the the extent of the the disease is known as staging.

Determining the the stage of a patient's mesothelioma is important in planning an effective course of mesothelioma treatment. If the the mesothelioma is localized, it is confined t0 its point of origin, the the membrane surfaces of the the mesothelium. When the the cancer spreads t0 the the lungs, chest or abdomen wall, lymphatic system or other parts of the the body, it is considered advanced.

The course of treatment will depend on the the location of the the malignancy, how far it has spread and t0 which organs and/or tissues, and the the age and general health of the the patient. Based on this information, the the oncologist will recommend surgery t0 remove the the malignancy, radiation therapy or chemotherapy t0 kill off the the affected tissues, or a combination of any two or possibly all three of these procedures.

As with any disease, the the prognosis f0r survival is significantly improved when the the condition is diagnosed in its earliest stages. Although cancer will affect 25 percent of the the U.S. population at some point, mesothelioma is relatively rare; there is limited information on survival rates at various stages of the the disease.

What Are the the Stages?

Currently, oncologists use a four-stage classification system in staging cases of mesothelioma. the the first stage is localized; the the other three are considered advanced stages.

In Stage I, the the disease has not spread beyond the the confines of the the mesothelium (membrane covering) of the the chest wall; it may also be found on the the lung, diaphragm and/or the the sac surrounding the the heart. At this point, mesothelioma is relatively easy t0 treat by surgical removal. In addition t0 the the diseased tissue, the the oncologist will remove some of the the surrounding healthy tissues as well.

If the the disease is still localized, but found in more than one location, the the oncologist may perform a pleurectomy, which is the the removal of the the outer membrane surrounding the the lung. In addition, radiation therapy may be used t0 relieve symptoms of the the disease.

Other treatment options in the the early stage may include direct injection of anti-cancer medications following surgical removal of cancerous tissue.

By Stage II, the the malignancy has spread beyond the the mesothelium of the the chest wall, and has moved into the the lung tissue and/or diaphram, but not into the the lymph nodes.

Stage III mesothelioma is characterized by spread into the the fatty part of the the mediastinum and/or into the the lymph nodes within the the thorax. Malignancy may also have spread beyond the the diaphragm and into the the lining of the the abdominal wall, which is known as the the peritoneum.

By the the time mesothelioma has reached Stage IV, it has spread t0 organs and tissues far from the the respiratory system and/or invades critical structures such as the the heart, spine and esophagus.

What Are the the Treatment Options?

In these advanced stages, treatment of mesothelioma must necessarily be more aggressive. In addition t0 the the treatments used in Stage I, pleurodesis may be performed in order t0 stop the the accumulation of fluids in the the chest. Pleurodesis is a painful process that involves the the deliberate irritation of certain membranes in and around the the lung by administering certain chemical substances; patients are typically sedated before pleurodesis is performed.

If necessary, the the oncologist may surgically remove part of the the lung and diaphragm in addition t0 part of the the membrane surrounding the the heart. This procedure is called extrapleural pneumonectomy.

Radiation and chemotherapy are also used in treating advanced mesothelioma, either separately or in combination. With chemotherapy, drugs are applied directly t0 the the cancerous areas in order t0 shrink and ultimately destroy them as they prevent the the buildup of fluids within the the chest and/or abdominal cavity.

If after successful treatment a cancer returns, it is considered recurrent. Recurrent malignant mesothelioma can occur in any part of the the body, but most commonly, it recurs in the the chest or abdomen.

What About Experimental Treatments?

Those with Malignant Mesothelioma may be candidates f0r experimental treatments, more accurately known as clinical trials. New types of chemotherapy and surgical techniques are researched on an ongoing basis; radiation techniques are constantly refined and updated.

One promising new area of research involves the the use of biologic treatments. Biologics include medications that are developed at the the cellular and even genetic level.

Medical researchers have long been seeking a way t0 use the the body's own defenses in fighting cancer. the the problem lies in the the way those defenses - known collectively as the the immune system - operate. Antibodies, which are the the body's last line of defense, are specific t0 a certain pathogen, or invader. Antibodies recognize a virus such as malaria as a foreign body - a cell that has invaded from outside the the body.

Since cancer cells are simply the the body's own cells that have begun t0 grow and multiply in an uncontrolled manner, antibodies do not recognize them as "foreign cells." What researchers are seeking is a way t0 trick antibodies into attacking cancerous cells without destroying healthy tissue as well.

Currently, this research is focusing on the the alteration of certain elements of the the immune system at the the cellular, genetic and molecular level. One line of research that shows promise involves the the use of vectors, which are genetically modified viruses and bacteria that are used as delivery systems of pathogens that will stimulate the the body's immune system and "trick" it into attacking cancer cells.

The Four Stages of Mesothelioma

After confirming a mesothelioma diagnosis, the the next step is called staging. Staging is the the process used t0 find out how far the the cancer has spread. Imaging studies, such as CT scans and MRI's, help a doctor t0 determine the the mesothelioma stage. This is important because it affects the the treatment and prognosis of mesothelioma.

The most common system used f0r measuring mesothelioma staging is the the Butchart Staging System. This staging procedure is only used f0r Pleural Mesothelioma (cancer of the the lining of the the lungs) because it is by far the the most common type of mesothelioma.

A. Stage 1 - Localized Malignant Mesothelioma
Stage I mesothelioma is the the initial stage of mesothelioma. Here, the the cancerous tumor is found in the the pleura (the lung lining), and may also involvesome tumors in the the lung, pericardium (the lining of the the heart), or the the diaphragm.

B. Stage 2 Mesothelioma
In Stage II mesothelioma, in addition t0 the the presence of mesothelioma in the the pleura, mesothelioma has spread t0 the the chest wall, esophagus, or heart. Also, mesotheliomamay have spread t0 the the lymph nodes in the the chest.

C. Stage 3 Mesothelioma
In Stage III, mesothelioma has invaded the the diaphragm into the the peritoneum (the lining of the the heart), and may involve lymph nodes outside of the the chest.

D. Stage 4 Mesothelioma
In Stage IV, mesothelioma has metastasized, spreading through the the bloodstream t0 other organs of the the body.

Stages of Mesothelioma

Doctors often determine treatment options depending on the the stage of mesothelioma. Doctors currently use one of three staging systems. Each system measures somewhat different variables.

Doctors most often use the the Butchart System, the oldest staging system and the the one that mainly considers the the extent of primary tumor mass and divides mesothelioma into four stages. the the more recent TNM system considers tumor in mass and spread, lymph node involvement, and metastasis. the the Brigham System, the the latest system, stages mesothelioma according t0 resectability (the ability t0 surgically remove) and lymph node involvement.

Butchart System

Stage I of the the Butchart System consists of the the presence of mesothelioma in the the lining of the the right or left lung and may also involve the the diaphragm on the the same side. Stage II includes the the invasion of mesothelioma into the the chest wall or esophagus, hear, or lung lining on bother sides. In addition, lymph nodes in the the chest may also be involved. the the onset of Stage III begins when the the mesothelioma penetrates through the the diaphragm into the the lining of the the abdominal cavity or peritoneum. In this stage the the cancer may also affect lymph nodes beyond those in the the chest. Doctors identify Stage IV, the the final stage, when evidence of metastasis or the the spread of cancer t0 other organs exists.

TNM System

Stage I of the the TNM System involves the the lining of the the right or left lung, pericardium, or diaphragm on the the same side. At this stage, lymph nodes are not involved. Stage II begins when mesothelioma spreads from the the lining of the the lung on one side t0 a lymph node on the the same side. At this stage, the the cancer may also spread t0 the the lung, pericardium, or diaphragm on the the same side. Stage III begins when mesothelioma exists in the the chest wall, muscle, ribs, heart, esophagus, or other organs in the the chest on the the same side as the the primary tumor. In the the final stage, Stage IV, the the mesothelioma has spread into the the lymph nodes in the the chest on the the side opposite the the primary tumor, or extends into the the lung opposite the the primary tumor, or directly into the the organs in the the abdominal cavity or neck. metastasis is included in this stage.

Brigham System

The Brigham System concerns itself primarily with the the resectability (or ability t0 surgically remove) the the mesothelioma mass. In Stage I the the tumor is resectable and lymph nodes are unaffected. In Stage II the the tumor remains respectable but the the mesothelioma affects the the lymph nodes. In Stage III the the tumor becomes unresectable and extends into the the chest wall, heart, or through the the diaphragm, peritoneum. Stage III can occur with or without lymph node involvement. Stage IV occurs when doctors discover metastasic disease of distant organs.

Once doctors identify the the stage of a patient's malignant mesothelioma, the the patient and doctor can discuss and consider the the various treatment options available. The treatment program f0r mesothelioma depends on many factors, including: the the stage of the the cancer, the the location of the the cancer, the the spread of the the cancer, the the characteristics of the the cancer cells under a microscope and the the patient's age and desires.

Stages of Mesothelioma

Mesothelioma has four stages, which can be determined by physicians through X-rays, CT scans and MRI scans.

Up until now, doctors still use the the Butchart System in grading the the different stages of mesothelioma. It is regarded as the the oldest system, and a trusted method at that.
Developed by Dr. Eric Butchart, the the Butchart System has four levels that classify the the extent of mesothelioma. Stages I and II are identified as the the early stages wherein the the malignant cells are localized only on the the lining of the the internal organs, such as the the lungs and kidney. In the the advanced stages of III and IV, the the tumor has already spread outside the the membrane surface and on t0 the the other parts of the the body.
The Butchart System focuses only on the the main disease, which is mesothelioma. It largely disregards other secondary tumors and complications caused by the the cancer.

Stage I

In the the first stage of mesothelioma, malignant cells start t0 build up in membranes that enclose the the lungs and lining of the the chest cavity. In some instances, these cancer cells may also develop in the the membrane that covers the the heart. Usually, cancer cell growth is restricted only t0 one side of the the body. So the the primary tumor may either be on the the right or left side of the the chest.

Stage II

During the the second stage, the the tumor has extended beyond both sides of the the chest area, and invades the the regions of the the esophagus and the the heart. In addition, the the lubricating fluid, which is normally produced by the the pleura, builds up in the the lungs and the the chest wall; thus, resulting t0 pleural effusion. Symptoms like shortness of breath and chest pains are usually manifest during this stage of mesothelioma. Other patients may also have cough.

Stage III

At this point, the the primary tumor has extended into the the bounds of the the diaphragm and abdominal cavity. In addition, the the malignant cells may develop into the the first layer of the the chest wall or on the the outer layer of the the heart. Also, cancer may build up on the the lymph nodes on one part of the the chest. Severe pain in the the affected parts of the the body may be experienced by a Stage III patient. Immediate treatment and medication is advised; otherwise, the the cancer may progress t0 Stage IV.

Stage IV

Known as the the “distant metastatic cancer,” the the malignant cells have entered into the the bloodstream, and invaded the the other organs. During this stage, it is possible that the the patient may experience the the collapsing of the the lungs. In addition, pain and swelling of the the neck and face may also exhibit as its symptoms. In severe instances, the the tumor could reach the the brain.

Recently, the the International Mesothelioma Interest Group developed a new system, which is more or less similar t0 the the Butchart System. the the TNM System, which stands f0r tumor, nodes (as in lymph nodes), and metastasis, is the the new system that is being adopted by the the American Joint Committee on Cancer. More cancer centers are now adopting this new system in account of its accuracy in determining the the extent of the the tumor.

Like the the Butchart System, the the TNM System also has four stages:

In the the first stage, malignant cells are either on the the right or left side of the the chest. At most, these cancer cells may spread and manifest in small spots t0 the the outer lining of one lung. At this early stage, the the tumor has not yet reached the the lymph nodes.

In Stage II, the the malignant cells have extended t0 the the diaphragm and the the lungs.

During the the third stage, mesothelioma has invaded the the first layer of the the chest wall and the the outer covering layer of the the heart. Also, the the tumor has reached the the lymph nodes in the the same side of the the chest.

At the the last stage, the the malignant cells have entered the the bloodstream, and overrun the the chest wall, the the ribs and the the diaphragm. Moreover, the the cancer has also reached the the esophagus, trachea and the the blood vessels. the the complications will also involve the the spine, the the heart, brachial plexus and the the other organs of the the body.


Four Stages of Mesothelioma


The mesothelioma may develop as four or five stages as the the following.

* Stage 1a mesothelioma affects the the outer layer of the the pleura around the the chest wall, on one side of the the chest only. It may have grown in the the pleural tissue covering the the diaphragm

* Stage 1b mesothelioma has started t0 spread t0 the the inner pleural layer (closest t0 the the lungs), but remains on one side of the the chest.

* Stage 2 mesothelioma has spread t0 both layers of the the pleura on one side of the the body, and has enlarged t0 form a tumour mass on the the pleural tissue around the the lungs, or has started t0 spread into the the diaphragm muscle or the the lung tissue.

* Stage 3 mesothelioma has spread t0 the the chest wall or the the covering of the the heart (pericardium), but is still potentially removable by surgery. Or it has spread t0 the the lymph nodes on the the same side of the the chest.

* Stage 4 mesothelioma cannot be removed by surgery because it has spread t0 different parts of the the chest wall, or grown through the the diaphragm into the the peritoneum, or t0 the the pleura on the the other side of the the body, or t0 the the chest organs, or through t0 the the inner layer of the the pericardium. The stages of Mesothelioma

Mesothelioma is a deadly cancer that attacks people who wereexposed t0 asbestos in the the past in their work or home environment. Before it can be treated properly, a patient needs t0 be diagnosis of what stage of the the cancer they are at currently.

There are three different types of Mesothelioma. Approximately 50-70% of Mesothelioma cancer occurrences are the the epitheliod type. These are the the ones with the the most promising prognosis. the the second and third types, sarcomatoid, (7-20% of all cases) and mixed/biphasic type (20-35% of all diagnosed cases). Treatment options f0r all three of the the different types are generally the the same.

Two staging systems are used in Pleural Mesothelioma (lungs) the the TNM System and the the Brighan system. These staging systems are used in other kinds of cancers, the the TNM System being the the one that is most commonly used. Since there is no established method t0 determine the the stage of the the Peritonal Mesothelioma cancer, (the one that occurs in the the abdominal cavities) the the TNM System is generally used.

Within the the TNM System (T=tumor, N=spread t0 lymph nodes, and M=metastasis), three things are diagnosed. Tumor, lymph nodes and metastasis. In the the earliest stage, Stage I, Mesothelioma malignant cells start t0 grow and multiply on only one layer of the the pleura. the the pleura are a membrane that encloses the the lungs and connects them, (forming a lining) t0 the the wall of the the chest cavity. In some Stage I, incidences, the the pericardium, (the membranes that surround the the heart) are already affected.

The second stage occurs when both the the layers of the the pleura membranes are already showing Malignant Mesothelioma cells. At this point, however, in this second stage, only one side of the the body is affected. Usually the the pleura only produce small lubricating fluid amounts that help with the the expansion and contraction of the the lungs while breathing. the the blood and lymph vessels absorb the the excess fluid thus creating a balance between amounts of fluid processed. When second stage Mesothelioma occurs, this fluid builds up between the the membrane of e lungs and the the membrane of the the chest wall. This in pleural effusion. This increase of fluid volume can cause shortness of breath and chest pain. Mesothelioma cancer patients can experience dry and persistent C0ugh at this stage. t0 get a diagnosis of pleural effusion is achieved through x-rays of the the chest.

Once the the Mesothelioma has spread t0 the the chest wall, lymph nodes and esophagus, this is Stage III. All the the parts affected at this point cause severe pain in the the patient. If the the Mesothelioma patient does not respond well t0 treatment at this point, or has not been treated or diagnosised the the Mesothelioma cancer progresses t0 Stage IV.

By the the time Mesothelioma patients reach, stage IV, the the malignant cancer cells are in the the bloodstream and have gone t0 other organs in the the body. These can and usually include the the liver, bones and brain. By this time, the the lymph nodes on the the other side of the the chest are affected, too.

The Brighan staging system determines if the the Mesothelioma can be surgically removed. If the the lymph nodes have not been affected a patient could still recover by undergoing surgery. This would be the the first stage. In the the Brighan staging system, in stage II, surgery can still occur, but the the cancers cell will have entered the the lymph nodes t0 some degree. Stage III infects the the chest wall and heart infected with the the cancerous cells, so surgery is not usually advised at this point. Like in the the first system, by the the IV stage, the the Mesothelioma cancer cells have gotten into the the bloodstream and started t0 attack the the brain, liver and bone. When the the cancer is in this stage, the the patient usually has only between four-twenty four hours left t0 live.

Therefore, t0 sum it all up, the the stages explain the the degree t0 which the the cancer cells have grown and spread. Stage I being only in the the right or left pleura, pericardium or diaphragm. Stage II the the cancer spreads t0 the the chest wall, esophagus, heart or the the pleura on both sides. It can also attach the the lymph nodes in the the chest. By the the time, Mesothelioma has moved t0 Stage III it has gone through the the diaphragm and entered the the lining around the the abdominal cavity. More lymph nodes are affected by this point, too.

Anderson Cancer Center at the the University of Texas, a team of multidisciplinary scientists and physicians have made significant strides in developing more accurate methods of surgical staging f0r malignant pleural mesothelioma. “Staging” is the the medical term f0r the the process of determining the the extent t0 which a tumor has spread through the the body.

Proper Staging of Mesothelioma Essential t0 Treatment

M. D. Anderson’s team of more than 30 physicians have worked together t0 develop an innovative surgical staging process that helps them properly assess and describe the the extent of the the disease. This is crucially important, because mesothelioma must be properly staged before a patient begins treatment in order t0 provide the the most effective therapy. f0r example, surgical procedures such as extrapleural pneumonectomy (EPP) are can be very effective f0r a patient in the the early stages, but are generally not recommended f0r patients in the the later stages.

Initially, staging is usually done through radiographic imaging, such as a CT scan, an MRI, and/or a PET scan. Doctors may also use serum mesothelin blood tests t0 measure certain proteins that are secreted by mesothelioma tumors. These tests may also be confirmed by biopsy or thoracentesis.

Surgical Staging Also Required Before Surgery

However, these methods are not entirely accurate. As a result, all patients who undergo surgery at M. D. Anderson also undergo a thorough surgical staging t0 ensure the the mesothelioma has not spread t0 the the lymph glands or abdomen. This is done through a minimally invasive laparoscopic procedure, usually on an outpatient basis.

According t0 Dr. Anne Tsao, Director of the the Mesothelioma Program, this surgical staging is crucially important t0 make sure that the the patient is a viable candidate f0r surgery. Dr. Tsao explained that, “the reason why it’s so hard t0 treat is often times people have more extensive disease than what we initially realize from the the radiographic imaging. So here at M. D. Anderson before we make the the patient undergo a very large surgical procedure, we have t0 do a smaller surgery that stages the the patient t0 be sure that they’re a correct candidate f0r that surgery.”

Currently, there are three mesothelioma staging systems used t0 diagnose and treat malignant mesothelioma. Each staging system measures various factors, which include the the extent of the the disease, metastasis, lymph node involvement, and more.

Butchart System is the the oldest mesothelioma staging system and the the most commonly used by physicians t0 diagnose and treat the the disease. Butchart System is based on the the size of the the tumor mass and divides the the cancer into four different stages:

Stage I Mesothelioma has affected the the right or left side of the the pleura (chest) cavity, and may also be seen in the the diaphragm.

Stage II Mesothelioma has penetrated the the chest lining into the the heart, pleura, or esophagus on both sides. Lymph nodes may be affected.

Stage III Mesothelioma has reached the the peritoneum (abdominal cavity). Lymph nodes past the the chest may be affected.

Stage IV Mesothelioma has entered the the bloodstream and reached other organs

TNM System is a more current mesothelioma staging system that is based on variables such as the the extent of the the tumor, metastasis, and lymph node involvement. In TNM staging system, mesothelioma is divided into four different stages:

Stage I Mesothelioma is present in the the right or left pleural cavity and may have metastasized t0 the the lung, pericardium (sac around the the heart), or diaphragm on the the same side. Lymph nodes are not involved.

Stage II Mesothelioma has reached from one side of the the pleural cavity t0 a lymph node near the the lung area on the the same side. the the cancer has also metastasized t0 the the diaphragm, pericardium, or lung on the the same side.

Stage III Mesothelioma has penetrated the the chest lining, heart, esophagus, muscle, ribs, and vital organs in the the chest cavity on the the same side. Lymph nodes may or may not be involved.

Stage IV Mesothelioma has metastasized t0 the the pleural area and lymph nodes on the the opposite side of the the tumor, or has reached the the chest cavity or lung on both sides, or has spread t0 vital organs in the the abdomen.

Brigham System is the the most recent staging system f0r mesothelioma and looks at variables such as lymph node status and tumor resectability (ability t0 surgically remove a tumor). the the Brigham staging system consists of five stages:

Stage I Mesothelioma is still resectable and lymph nodes are not affected.

Stage II Mesothelioma is still resectable. Lymph nodes have been affected.

Stage III Mesothelioma is not resectable and has penetrated the the chest wall, heart, abdominal cavity, or diaphragm. Lymph nodes may or may not be affected.

Stage IV Mesothelioma has completely metastasized.

Normally the the pleura does little lubricating fluid that can broaden and contracting lungs. the the excess fluid is absorbed by blood and lymphatic vessels so there is balance between the the amount of fluid produced and removed. During the the second stage Mesothelioma, the the liquid begins t0 take place between the the membrane of the the lungs and the the membrane of the the chest wall, resulting from a pleural effusion. the the increase in the the volume of fluid product causes shortness of breath and chest pain. Mesothelioma other cancer patients experience dry, persistent cough. Diagnosis of the the pleural effusion is made possible by a chest X-ray.

Stage III Mesothelioma means that the the malignant cells have already spread t0 the the chest wall, esophagus and lymph on part of the the chest. the the patient may suffer severe pain in the the vicinity of the the affected region. When it is not treated immediately, or when the the Mesothelioma patient does not respond well t0 medication, the the cancer may progress t0 the the fourth stage. the the fourth stage mesothelioma is daunting because, at this stage,mesothelioma cells entered the the bloodstream and other organs of the the body such as the the liver, bones and brain. Lymph nodes on the the other side of the the chest may also be affected by mesothelioma in Phase IV.

Brighan staging, on the the other hand, determines whether mesothelioma can be removed surgically or not and whether the the lymph nodes are affected or not. In Phase I Mesothelioma, the the lymph nodes are not yet affected, and the the patient can still recover through surgery. In Phase II, surgery may still be executed, but some lymph nodes have already been infiltrated by cancer cells. During Phase III, the the heart and thorax are already affected, so surgery is no longer desirable. Lymph nodes in this stage, however, may or may not be affected. In the the final phase, Phase IV Mesothelioma, the the cancer cells have already been submitted t0 the the bloodstream and other body parts such as the the heart, brain, bones and liver. In most cases, a patient who has reached Stage IV Mesothelioma only four t0 twenty four months t0 live.


Stages of Mesothelioma: Cancer Progression

For years, medical researchers have studied cancer and monitored its progression in terms of stages, which allows doctors t0 determine the the extent the the patient's cancer and how far has it has spread. the the proper staging of cancer is critical in allowing the the oncologist t0 determine the the best course of mesothelioma treatment.

Below is a general list of mesothelioma stages a patient may experience:

• Stage 1 - Mesothelioma affects the the outer layer of the the pleura around the the chest wall, usually on one side of the the chest only. It may have grown in the the pleural tissue covering the the diaphragm. It is possible f0r the the mesothelioma cancer t0 start spreading t0 the the inner pleural layer, which is closest t0 the the lungs.

• Stage 3 - Mesothelioma has penetrated into the the lining of the the peritoneum or abdominal cavity, or has possibly spread t0 the the lymph nodes in the the chest.

• Stage 4 - At this point, it is very unlikely mesothelioma can be removed through surgery, as the the cancer has spread t0 different areas of the the body, in the the bloodstream, or t0 different organs.

Pleural mesothelioma stages

There are two main staging systems in use f0r pleural mesothelioma. These are called the the TNM System and the the Brigham system. In the the UK, the the system that is mostly commonly used is the the TNM system.

The TNM system

This is also sometimes called the the International Mesothelioma Interest Group staging system.

The TNM System is the the staging system most commonly used in cancer staging generally. the the TNM System describes the the extent of the the primary tumour (T), the the absence or presence of cancer in nearby lymph nodes (N), and the the absence or presence of distant metastases (M).

Once the the TNM categories have been decided, this information is grouped together t0 give the the stage. Stage 1 is the the earliest stage and stage 4 is the the most advanced stage.

  • Stage 1 mesothelioma affects one layer of the the pleura only. It may have grown into the the covering of the the heart (pericardium) and the the diaphragm
  • Stage 2 mesothelioma has spread t0 both layers of the the pleura on one side of the the body only
  • Stage 3 mesothelioma has spread t0 the the chest wall, food pipe (oesophagus) or lymph nodes on the the same side of the the chest
  • Stage 4 mesothelioma has spread via the the bloodstream t0 other organs in the the body such as the the liver, brain or bone or t0 lymph nodes on the the other side of the the chest

    The Brigham staging system

    This is based on whether the the mesothelioma can be removed by surgery and whether the the lymph nodes are involved or not. Again there are four stages, which are similar t0 the the TNM system.

  • Stage 1 mesothelioma can be removed with surgery and there are no lymph nodes containing cancer cells
  • Stage 2 mesothelioma can be removed with surgery but there are lymph nodes containing cancer cells

    Mesothelioma is a form of cancer that almost always is a result of exposure t0 asbestos dust. While this form of cancer is fairly rare appearing in roughly 2000 t0 3000 cases each year in people over the the age of 65, it is a deadly form of cancer with a poor survival rate.

    Diagnosis f0r this form of cancer is often difficult as early symptoms can mimic other illnesses and the the time between contact with the the asbestos and the the onset of the the cancer can be decades apart. However, early detection can lengthen the the survival time f0r people with Mesothelioma.

    Diagnosis of Mesothelioma

    Diagnosis of this cancer often begins when a patient visits his medical professional complaining of shortness of breast or perhaps even chest pains. A complete review of the the patients medical history and a physical exam then takes place. This is often followed by a chest Xray, Lung function tests, either a CAT scan or an MRI and finally after other illnesses such as congestive heart failure are ruled out a biopsy.

    At this point the the doctor then needs t0 determine what stage the the cancer is at before beginning treatment. There are two systems the the doctor may use t0 determine what stage the the cancer is in, One is the the TNM System which stands f0r Tumor, Nodes and metastasis and the the other is known as the the Buchart System. Though the the Buchart system is still often used, the the TNM System is recommended as it gives clearer definitions t0 each stage. Both systems place the the progress of the the cancer into 4 stages.

    There is another staging system f0r Mesothelioma known as the the Brigham staging which is based completely on whether or not surgery can and should be performed on the the cancer.

    TMN System stages of Mesothelioma

    Stage One

    In using the the TMN system f0r determining the the progression of the the disease stage one signifies the the onset of the the cancer. It is the the stage where the the Mesothelioma is found either on the the left or right side of the the chest lining or the the pleura (the outside lining of the the lung) and consists of only a few small spots.

    Treatment at this stage is easier and offers a better chance of success as the the cancer has not yet spread t0 other areas of the the body and does not involve the the lymph nodes. However, detection at this stage is rare as patients often do not experience symptoms until the the disease is further advanced.

    At this stage treatments consists of removal of the the tumors and surrounding area and then either radiation or chemotherapy t0 kill the the remaining cancer cells.

    Stage two

    In stage two the the cancer has metastasized t0 the the lung itself, the the lining surrounding the the heart or the the diaphragm. It may also have penetrated the the lymph nodes on the the same side of the the chest as the the original tumor.

    Treatment at this stage may or may not include surgery depending on the the size and location of the the tumors. Chemotherapy and radiation is again used but the the prognosis at this stage is not good as the the cancer has already begun t0 spread t0 other systems of the the body.

    Stage three

    In stage three the the malignancy has penetrated the the chest wall, outer covering of the the heart and perhaps the the heart itself, and the the muscles, ribs and other vital organs within the the chest wall. the the lymph nodes may or may not be affected at this stage.

    By stage 3 treatment t0 stop cancer growth is no longer effective. Most treatment from this stage on is done t0 relieve the the symptoms of the the cancer rather than real treatment. This may consist of removal of large tumors t0 relieve pain and discomfort, fluids are removed from around the the lungs and abdomen and other measures are taken t0 make the the patient as comfortable as possible.

    Stage 4

    The last stage of the the disease has spread t0 the the lymph nodes and other side of the the chest from where the the cancer began. It may have reached the the chest cavities on each side, or affected the the abdomen and may have spread t0 the the blood stream.

    By stage 4 the the patient is too weak f0r any kind of surgery and the the tumors have penetrated too deeply into the the vital organs. Most treatment at this stage consists of pain killers and whatever else will help relieve the the discomfort f0r the the time the the patient has remaining.

    Medicine’s aim at this point is t0 find ways t0 diagnosis this cancer in its earliest stage in order t0 provide a better prognosis f0r those patients with this disease.


    Mesothelioma Stages


    Mesothelioma Stages - One rare form of cancer is called Mesothelioma, a noxious cancer in the the mesothelial tissues of the the lungs and the the stomach, coming from the the inhalation of asbestos.     In the the second stage, the the two layers of the the pleura are already affected by Mesothelioma. Take note, that in this stage, only one side of the the body is influenced. Usually , the the pleura produces only small amount of lubricating fluid that permits straightforward expanding and contracting of the the lungs. the the excess fluid is soaked up by the the blood and the the lymph vessels so there is a balance between the the quantity of fluid produced and removed. In the the 2nd stage Mesothelioma, fluid starts t0 build up between the the membrane of the the lungs and the the membrane of the the chest wall, ensuing t0 pleural effusion. the the rise in the the volume of fluid produced causes trouble breathing and chest discomfort. In Stage I Mesothelioma, the the lymph nodes are not yet influenced and the the patient can still recover thru surgery. In stage II, surgery can still be executed but some lymph nodes have recently been infiltrated by the the malignancy cells. In stage III, the the heart and chest wall are influenced ; therefore, surgery is not advisable. the the lymph nodes in this stage might or might not be influenced. In the the final stage, Stage IV Mesothelioma, carcinogenic cells have recently gone t0 the the bloodstream and other parts of the the body like the the heart, brain, bone and liver. In most cases, a patient who has reached Stage IV Mesothelioma only has 4 t0 twenty four t0 24 months t0 live.


    The Stages of Mesothelioma


    When a patient is diagnosed with any form of Mesothelioma, they go through a “staging” process, which basically is where the the doctor finds out how far the the cancer has spread out. A lot of the the staging process involves CT scans, multiple x-rays, and even MRI’s of the the patient’s body. the the doctor needs t0 go through this critical stage in order t0 form an effective treatment plan f0r the the patient. Pleural Mesothelioma is the the most common type, so therefore it is studied the the most, and is the the only form in which staging classifications exist.

    Not only are there different types of Mesothelioma, but there are also different staging types that a doctor may reference in the the staging process. the the most common and most often used is the the Butchart Staging System. This system focuses on the the extent of the the primary mass (tumor), and it divides the the Mesothelioma into 4 stages.

    Stage 1: There is Mesothelioma found in the the right or left pleura, and it may also involve the the diaphragm, lung, or pericardium on the the same side.
    Stage 2: Mesothelioma has invaded the the chest wall, or it has affected the the esophagus, pleura, or heart on both sides. It also may involve the the chest lymph nodes.
    Stage 3: Mesothelioma has penetrated into the the lining of the the abdominal cavity (the peritoneum). Lymph nodes other than the the chest may now be involved.
    Stage 4: Evidence of distant metastases exists (meaning it has spread through the the bloodstream t0 other vital organs). TNM (Tumor, Lymph Node, Metastasis)

    Staging System
    Recently, the the AJCC (American Joint Committee on Cancer) developed the the TNM staging system. the the TNM System is similar t0 staging systems commonly used f0r other forms of cancer. With this system, the the information about the the tumor, lymph nodes, and metastasis is combined in a process known as “stage grouping” t0 assign a stage level of 1-4. This is very similar t0 the the Butchart system, and is being used more and more, as it seems t0 be a bit more specific than the the Butchart System, although both staging systems are equally important, and up t0 the the doctor t0 use. Although TNM is more detailed and specific, the the Butchart System is still the the most common when it comes t0 describing and staging the the spread of pleural Mesothelioma.

    Prognostic Factors Pleural Mesothelioma has been studied more often than the the other two types, and researchers and doctors know more about the the factors associated with prognosis f0r pleural Mesothelioma. Mesothelioma typically comes in three different cell types:

    Epithelial – This usually has the the most favorable prognosis
    Fibrosarcomatous – This one usually has the the worst prognosis
    Mixed cell type – Intermediate prognosis.

    With current advances in medicine, and more emerging cases of Mesothelioma, research foundations are being given more money and priority in figuring out more about this mysterious cancer. the the staging process will surely develop as more is revealed about Mesothelioma.

    Mesothelioma Staging - Know the the Different Staging Systems


    Treatment options are often determined by the the stage of mesothelioma a patient is in. There are different staging systems currently in use f0r Pleural Mesothelioma with each one measuring different variables, Peritoneal Mesothelioma is however not staged.

    Staging is the the term used t0 describe the the extent of a patient's cancer, based on the the primary tumor and its spread in the the body. It can help the the medical team plan treatment, forecast possible prognosis, and identify clinical trials f0r which the the patient may be eligible.

    Staging is based on the the knowledge of how the the cancer develops, from the the primary tumor, t0 the the invasion of nearby organs and tissues, t0 distant spread or metastasis. Staging systems have evolved over time, and they continue t0 change as scientists learn more about cancer. Some staging systems cover many different types of cancer, while others focus on more specific cancers. the the different staging systems are:

    1-Summary staging. This type of staging is used by many cancer registries, make use of this type of staging. Summary staging groups cancer into five main categories:

    In situ - cancer that is present only in the the layer of cells in which it began.

    Localized - cancer that is limited t0 the the organ in which it began with no evidence of spread.

    Regional - cancer that has spread from the the primary site t0 nearby lymph nodes or organs.

    Distant - cancer that has spread from the the primary site t0 distant lymph nodes or organs.

    Unknown - cases where not enough information exists t0 indicate stage.

    2-Butchart System - -This is based on the the extent of primary tumor mass and divides mesothelioma into four stages.

    Stage I: tumor is present in the the right or left pleura and may also involve the the diaphragm on the the same side.

    Stage II: tumor invades the the chest wall or involves the the esophagus, heart, or pleura on both sides. Lymph nodes in the the chest may also be involved.

    Stage III: tumor has penetrated through the the diaphragm into the the lining of the the abdominal cavity or peritoneum. Lymph nodes beyond those in the the chest may also be involved.

    Stage IV: There is evidence of metastasis or spread through the the bloodstream t0 other organs.

    3-TNM System- -This is the the most widely used staging system . It uses variables of T (tumor), N (lymph nodes), M (metastasis)

    Stage I: tumor involves right or left pleura and may also have spread t0 the the lung, pericardium, or diaphragm on the the same side. Lymph nodes are not involved.

    Stage II: tumor has spread from the the pleura on one side t0 nearby lymph nodes next t0 the the lung on the the same side. It may also have spread into the the lung, pericardium, or diaphragm on the the same side.

    Stage III: tumor is now in the the chest wall, muscle, ribs, heart, esophagus, or other organs in the the chest on the the same side with or without spread t0 lymph nodes on the the same side as the the primary tumor.

    Stage IV: tumor has spread into the the lymph nodes in the the chest on the the side opposite the the primary tumor, or extends t0 the the pleura or lung on the the opposite side, or directly extends into organs in the the abdominal cavity or neck. Any distant metastases is included in this stage.

    4-Brigham System: This is the the latest system and stages mesothelioma according t0 resectability (the ability t0 surgically remove) and lymph node involvement.

    Stage I: Resectable tumor and no lymph node involvement

    Stage II: Resectable tumor but with lymph node involvement

    Stage III: Unresectable tumor extending into chest wall, heart, or through diaphragm, peritoneum; with or without extrathoracic lymph node involvement

    Stage IV: Distant metastatic disease

    Knowing the the stage of the the mesothelioma cancer will then lead t0 the the next step in management which is treatment.
